Consumer Access to Health Records and Complaints

This page is provided in compliance with Texas House Bill 4224 (HB 4224) and Section 181.105 of the Texas Health and Safety Code, relating to consumer access to health care records.

1. How to Request Your Health Care Records

You have the right to access your protected health information. To request a copy of your records from Dr. Kuhns’ practice:

• Contact our office directly by email at info@drkuhns.com.
• Submit a written request via mail to: 7019 W. Village Blvd. Set 201, Laredo, Texas 78041.

We will respond to your request within 15 business days (or as required by law). A reasonable, cost-based fee may apply for copies. We cannot deny access due to unpaid bills for services.
